Quality Improvement Award
Million Hearts Champion
The health center was recognized as a Million Hearts Hypertension Control Challenge 2018 Champion! NH Health Center was the ONLY Federally Qualified Health Center from California to receive this award. We take pride in putting our patients first and it is our goal to help our patient towards the steps of living a happy and healthy life.
2018 Annual Health Fair
Students from NH Health Center’s Tai Chi class perform for the audience at NH Health Center’s 2018 Annual Health Fair. Free Tai Chi classes are held on Monday, Wednesday, and Friday from 9:30 am to 11:30 am. Students learn to relax in action, release stress, unite mind with body, develop self-control and self-confidence, improve concentration, and build stamina and strength.
CMS Transforming Clinical Practice Initiative
Nhan Hoa added the Loop Electrosurgical Excision Procedure (LEEP) treatment to its lineup of services. LEEP is a treatment that helps in the prevention of cervical cancer by removing abnormal cells from the cervix.
Expansion of Optometry and Dentistry
Optometry received a roomy expansion and new medical equipment while Dental increased its number of chairs to four.
Top Medical Home Award
Orange County and the Medical Services Initiative (MSI) program recognized Nhan Hoa as a top medical home in providing quality care in the area of Family Medicine.
Grand Opening 7761 Garden Grove Blvd
Nhan Hoa began offering Pediatric care.
Tobacco Cessation Program
Nhan Hoa introduced its Tobacco Cessation Program which focused on guiding patients through their journey to quit smoking. Tobacco cessation educational seminars and individual counseling sessions were available, and participants received free nicotine patches. The Tobacco Cessation Program is still active through the Health Education department.
Women’s Cancer Support Group
A group of ten volunteers established the Cancer Support Group in order to assist the community in coping with losses due to cancer. Since then, Nhan Hoa’s Health Education has continued to host support group meetings where patients can discuss their experiences, share ideas, and provide emotional support for one another. Additionally, we have guest speakers to help facilitate discussions on relevant health topics while enjoying a complimentary lunch.
Car Seat Safety Program
After introducing Prenatal Care classes, Nhan Hoa began a “Car Seat Safety” campaign aimed at increasing community safety through guidance and distribution of car seats to the class’ attendees. The campaign has since developed into a program offered through the Health Education department.
Prenatal Care classes are offered through 5-day series to assist parents during their pregnancy. Nhan Hoa continues to offer Prenatal Care classes through the Health Education department and guides parents on topics such as:
- Nutrition During Pregnancy & Eating for Two
- Preparing for Labor and Delivery
- Taking Care of the Newborn & Breastfeeding
- Car Seat Safety
The Behavioral Health Department is established and staffed by volunteers.
In-House Pharmacy is Introduced
Nhan Hoa opened a pharmacy on the premises to provide patients access to their prescriptions right at the facility.
Nhan Hoa began offering health education classes. Our health educators facilitated classes to teach patients about chronic diseases and how to manage their chronic conditions. Classes on diabetes awareness were available and guided patients on topics such diabetes prevention and diet and nutrition.
Women’s Health became an official department with mammography screening services provided through a breast cancer awareness program. Nhan Hoa collaborated with the Vietnamese American Cancer Foundation (VACF) to help women detect breast cancer through screening and breast health education.
The Fourth Clinic
When the Chapman office began to reach max capacity, Nhan Hoa relocated to a new 2,400 square foot facility on Euclid Street. This location had expanded to 8,500 square feet by 2004, and served as Nhan Hoa’s base of operations for 17 years. Thanks to the increase in space, Nhan Hoa began making significant progress in developing and implementing new programs and services.
The Third Clinic and the Coalition
The clinic relocated to a 1200 square foot office on Chapman Avenue, Garden Grove. Soon after our relocation, the number of patients increased significantly. In addition, Nhan Hoa became a member of the Coalition of Orange County Community Health Centers.
Optometry became the second official department and was operated by a single volunteering Ophthalmologist.
The Second Clinic
Nhan Hoa moved to a larger 700 square foot office located on Magnolia Avenue in Westminster. The volunteer doctors continued to provide free healthcare services to the community. This office was spacious compared to the previous one; however, one of the problems was the high cost of rent. Each doctor contributed $1,000, and Nhan Hoa was able to continue serving the uninsured. During this time, the volunteers formed the Board of Directors and elected Dr. Vien Doan as the Chairman, Dr. Thua Vinh as the Vice Chairman, Kiet Truong as Secretary, Hanh Dao as Treasurer, and Dr. Thong Vu as Medical Director.
The First Clinic
A small group of volunteers opened the first of Nhan Hoa’s medical practices, a small 300 square foot office located on Lake Street, Garden Grove. Four doctors dedicated 8 to 12 hours of their time to providing healthcare services to the uninsured patients. Through word of mouth, the number of patients gradually increased along with the number of volunteers.
