FREQUENTLY ASKED QUESTIONS:
1. How do I make an appointment?
To make an appointment call 714-898-8888. Please tell the call center operator what services you need.
2. How soon will I receive my appointment?
We will do our best to work around your schedule and make an appointment that is convenient for you. Nhan Hoa clinic is open from 8:00 a.m. to 8:00 p.m. Monday through Friday. On Saturday from 8:00 a.m. to 1:00 p.m. Please note we are closed daily for lunch, from 1:00 p.m to 2 p.m.
3. I have an appointment scheduled. What do I need to bring on the day of my visit?
On the day of your appointment, please arrive 15 minutes early to register. Please bring the following information that is required to complete your patient registration:
a) a valid ID such as:
- Driver’s license
- State identification
- Matricula card
- Employment or school ID
- Public aid ID
- Social Security card
- Alien registration card/”green card”
- Current utility bill
- Valid passport
- Veteran’s ID
b) your current address and phone number
c) medical card or insurance card (if you have one)
d) If you don’t have a medical card or insurance, please bring income information, such as:
- Two paycheck stubs
- W-2 form
- Medicaid card
- Food stamp card
- Unemployment notice
4. I need after hours care. What do I do?
If you have a medical emergency, please call 911 or go to your nearest emergency room. If you have a health concern that is not an emergency, our call center is open 24 hours a day, seven days a week. An on-call NHC doctor will help you if you call our general number: 714-898-8888. For all other business, including test results, scheduling an appointment and medication refills, please call during our regular business hours.
5. How do I cancel an appointment?
If you are unable to keep your appointment, please call 714-898-8888 as soon as possible so we can reschedule your visit and offer your time slot to another patient.
6. How do I request my medical records?
The privacy and security of your medical records is a priority. If another medical facility or hospital is requesting a copy of your records, you must come in person to the health center where you receive care. Bring a valid driver’s license or state ID and ask to speak to a Health Information staff person. You will be asked to complete and sign a medical release form which gives us permission to release your health information to other designated facilities. If you are a health facility requesting the health records of a Nhan Hoa Clinic patient, you can fax your request and a copy of the patient’s identification to: 714-901-7580

7. Does Nhan Hoa Clinic accept health insurance?
Nhan Hoa Clinic is currently participating with the following health plans:
- Cal-Optima (ARTA, AMVI, Noble-Mid, Tabert, UCMN)
- Commercial (ARTA, Noble AMA, Prospect Medical, Family Choice, Regal, HCP, CCN, HealthNet, Anthem Blue Cross and ADOC)
In addition, all pregnant women who do not have insurance are eligible for California Medi-Cal programs. Our staff can help you complete Health Covered applications. If you are already a Medi-Cal patient, you should enroll in Covered California and choose Nhan Hoa Clinic as your primary care provider.
8. I don’t have health insurance. Can I still be a patient at Nhan Hoa Clinic?
It is our mission to provide quality healthcare to all, regardless of your ability to pay. If you do not have insurance, we have many programs to help you. We accept cash, checks and credit cards at the time of your visit, and we can help you with a flexible payment plan that is right for you.
Patients who do not have insurance may qualify for our discount fee program, which is based on your household income and family size. Depending on your situation, you may save 25% to 100% off our regular fees. For more information about the program, please ask any front desk staff member.
