How do I make an appointment?​

To make an appointment call 714-898-8888 and download a registration form. Please tell the call center operator what services you need. 

How soon will I receive my appointment?

We will do our best to work around your schedule and make an appointment that is convenient for you. NH Health Center is open from 8am-8pm Monday through Friday and 9am-1pm on Saturday. 

Please note we are closed daily for lunch from 1pm-2pm.

What do I need to bring on the day of my visit?​

On the day of your appointment, please arrive 15 minutes early to register. Please bring the following information that is required to complete your patient registration:

  1. A valid ID such as:
    • Drivers’ license
    • State identification
    • Valid Passport
    • Matricula card
    • Employment or school ID
    • Public aid ID
    • Social Security card
    • Alien registration card/”green card”
    • Current utility bill
    • Veteran’s ID
  2. Your current address and phone number
  3. Your medical card or insurance card (if applicable)
  4. If you don’t have a medical card or insurance, please bring income information such as:
    • Two paycheck stubs
    • W-2 form
    • Medicaid card
    • Food stamp card
    • Unemployment notice
  5. If this is your first visit, medical records and medication lists are not required but highly encouraged
How do I cancel my appointment?

In the event that you are unable to keep your appointment, please call (714 898-8888) as soon as possible so we can reschedule your visit.

I need after-hours care. What do I do?

If you have a medical emergency, please call 911 or go to your nearest emergency room. If you have a health concern that is not an emergency, our call center is open 24 hours a day, seven days a week. An on-call NH doctor is available to help you if you call our general number: (714)-898-8888. For all other business, including test results, scheduling an appointment and medication refills, please call during our regular business hours.

What are the health insurance plans NH Health Center accepts?

NH Health Center is currently participating with the following health plans:

  • Medi-Cal/CalOptima
  • Medicare
  • Commercial (HMO/PPO)

For a comprehensive list of insurance we accept, you can check our insurance page or search for a provider participating with your insurance plan.

All pregnant women who do not have insurance are eligible for California Medi-Cal programs. Our staff can help you complete the Health Covered applications. If you are already a Medi-Cal patient, you should enroll in Covered California and choose NH Health Center as your primary care provider.

I don’t have health insurance. Can I still be a patient?

Our mission is to provide quality healthcare to everyone regardless of the ability to pay. If you do not have insurance, we have many programs to help you. We accept cash, checks and credit cards at the time of your visit, and we can help you with a flexible payment plan that is right for you.

Patients who do not have insurance may qualify for our fee discount program based on household income and family size. Depending on your situation, you may be eligible to save 25% to 100% off our regular fees. For more information about the program, please ask any front desk staff member.

How do I request medical records?

The privacy and security of your medical records are our priority. If another medical facility or hospital is requesting a copy of your records, you must come in person to the health center where you received care. Please bring a valid driver’s license or state ID and ask to speak to a Health Information representative.

You will be asked to complete and sign a medical release form which gives us permission to release your health information to other designated facilities. If you are a health facility requesting the health records of a NH Health Center patient, you can fax your request and a copy of the patient’s identification to: (714) 901-7580

Please note there may be a fee associated with some release requests. For more information, call (714) 898-8888 and ask for a Health Information representative.
